Dr. Kartik Iyer, MD

Dr. Kartik Iyer, MD, a board-certified Family Medicine Physician who recently relocated with his family from Arizona. As former Chief Resident in family medicine, Dr. Iyer brings exceptional expertise and heartfelt compassion to your care. His extensive experience as a hospitalist managing critical patients in the ICU also equips him to guide you through any health challenge with confidence.

Dr. Iyer is deeply dedicated to family medicine, delivering comprehensive and tailored care that prioritizes your unique needs. His focus on preventive care, weight loss management, and anti-aging solutions empowers you to feel energized and thrive. In his free time, he enjoys spending time with his family and exploring their new town of Apex.

Shreaya Patel, AGNP, PMHNP

Shreaya is passionate about helping people feel their best—inside and out. With a background in both primary care and aesthetics, she blends medical expertise with an artistic touch to enhance her patients’ confidence and well-being. She earned her Adult-Gerontology Nurse Practitioner degree from the University of Alabama at Birmingham and her Psychiatric-Mental Health Nurse Practitioner degree from the University of Cincinnati, allowing her to take a truly holistic approach to care.

 

As a mother of two, Shreaya understands the importance of balance in health, beauty, and everyday life. When she’s not caring for patients, you’ll likely find her outdoors, traveling to new places, or spending quality time with her family.

R. Madison McKoy, DNP, FNP-C

Madison is a people person who prioritizes making those around her feel heard, supported, and understood. This motivated her to choose a career in healthcare, leading her to becoming a registered nurse (RN) after earning her Bachelor's of Science in Nursing from the University of North Carolina - Wilmington. After gaining a few years of experience working in the emergency room and medical intensive care unit, she decided to continue her studies and become a nurse practitioner (NP). She earned her Family Nurse Practitioner degree from the Medical University of South Carolina in Charleston, where their primary care focused program prepared her to provide holistic and patient-centered care to patients of all ages and walks of life. For the majority of her NP studies, Madison trained at a Direct Primary Care (DPC) clinic and experienced first hand how the practice model worked to restore the provider-patient relationship through accessible, personalized, and transparent healthcare. She was so inspired by this, that after graduation she sought to continue practicing using this model to best serve and support her community.

 

When she is not caring for her patients, you will find Madison walking nature trails with her dogs (Piper and Eleanor), planning mini get-aways to make the most of her weekends and time off, or reading a book while enjoying a latte at a local coffee shop.
